Name, Social Security Number (SSN) and/or DoD ID Number, home address, phone number, work e-mail address, gender, date of birth, unit assigned; medical readiness data that includes immunizations (dates and type), laboratory results and/or procedures, physicals, eye, audio and dental exams, dental readiness category and injury status.

PURPOSE(S):

The purpose of this system of records is to track medical readiness to ensure individuals are medically eligible to be deployed; to track labs, exams, physicals, and immunizations that impact medical readiness; and to provide immunization data to the Defense Enrollment Eligibility System (DEERS).

ROUTINE USES OF RECORDS MAINTAINED IN THE SYSTEM, INCLUDING CATEGORIES OF USERS AND THE PURPOSES OF SUCH USES:

In addition to those disclosures generally permitted under 5 U.S.C. 552a(b) of the Privacy Act of 1974, as amended these records contained therein may specifically be disclosed outside the DoD as a routine use pursuant to 5 U.S.C. 552a(b)(3) as follows:

The DoD Blanket Routine Uses that appear at the beginning of the Navy's compilation of system of record notices may apply to this system.

NOTE: This system of records contains Individually Identifiable Health Information. The DoD Health Information Privacy Regulation (DoD 6025.18-R) issued pursuant to the Health Insurance Portability and Accountability Act of 1996, applies to most such health information. DoD 6025-18-R may place additional procedural requirements on the uses and disclosures of such information beyond those found in the Privacy Act of 1974 or mentioned in this system of records notice.

Paper file folders and electronic storage media.

Name and SSN.

SAFEGUARDS:

Computer processing facilities and terminals are located in restricted areas accessible only to authorized persons that are properly screened, cleared, and trained. Access to this system of records and personal information is restricted by use of the Common Access Card (CAC). Manual records and computer printouts are available only to authorized personnel having a need-to-know.

RETENTION AND DISPOSAL:

Disposition pending. Until the National Archives and Records Administration has approved a records retention, these records will be treated as permanent.
